* Work notes provided at the discretion of the physician. Short/long term disability and/or prolonged periods of time off will not be provided and patients are encouraged to see their in person healthcare provider for this request.

Note: we do not treat bipolar disorder, schizophrenia or ADHD. Please ask about the option for a referral to a psychiatrist or please see an in person healthcare provider.

About mental wellness

  • Our healthcare providers can help with anxiety, depression or feeling sad, difficulty with sleeping, loneliness, stress management, worry and quitting smoking.

  • If you are not feeling like yourself (more often than not) or if you have been feeling down, unable to sleep or feeling on edge then it may be time to speak with a healthcare provider. Mental healthcare prevention is also important, so if you need additional support, boost in the right direction or want to know what resources are available to you, let us know.

  • Yes. We can assist with care coordination and help you with a referral to a psychiatrist.

  • Yes, doctor's notes may be provided at the discretion of the physician. Doctors notes will not be provided for extended periods of time off and will not be provided for disability or employment insurance claims (such as Ontario works). Please see an in person physician for these concerns.
